Abstract

Texas City Refinery accident in March 2005 created important learning opportunities in the areas of Process Safety Culture and Occupational Health and Safety Risk Management. Accident studies and independent expert opinions have concretely shown that a systematic approach and integrated management system view are important in developing a culture of major accident prevention and safety for an organization.

In this study, the visible and root causes of the major industrial accident that occurred in the refinery were examined, the results of the occupational accident were examined and reactive suggestions for the workplace were investigated. Thus, the importance of occupational health and safety practices in integrated management systems is emphasized, and the basic principles for achieving safety leadership and safe behaviors are also summarized
